Sending Intake Forms to Patient

To send intake paperwork to patients, you will go to the Patients tab and select the Edit Pencil of the patient > Select Actions Menu at the top > Send Intake Link

Select the forms and outcomes you want to send to the patient by selecting and using the single arrow in the middle:
  • Red: Referring Provider - Who referred the patient
  • Orange: Rendering Provider - The provider the patient will be seeing in office
  • Green: Intake forms - The intake forms you want to send will need to be on the Right side of the shuttle
  • Purple: Outcomes - The outcomes you want to send will need to be on the Right side of the shuttle

You will be able to email and/or text the patient their paper work.

Once you have initially sent the intake paperwork you will be able to see the most recent Intake code on file for the patient to access their paperwork:

This can be accessed from the Patients Profile > Details > Actions > Send Intake Link:
