Set Up Insurance

Click "Billing" > Click "Insurance" > Click "New" > 

Fill in all boxes with a red asterisk(any other info you want) Type the "Company Name" > Select "Reimbursement" option (if there is a flat rate, select that option and enter flat rate price in the box) > Select "Billing Guidelines" > Select "Category" > Determine "Auth Required" > 

Enter "Claims Address" > Enter "Claim Phone #" > Click "Create Company" >
